This is the standard AdoptIQ Order Form template. It is Schedule 1 to the Master Services Agreement and sets out the commercial terms specific to each Customer. Print or save as PDF to use.
1. Parties

2. Effective date and term

Initial term: 12 months from the Effective Date, auto-renewing for successive 12-month periods unless either party gives 60 days' written notice.
